Please note: This is a template document which Shared Lives schemes can use to create their own policies and procedures. This is not the Shared Lives Plus complaints policy.
If you are unhappy about the quality or level of service provided by Shared Lives Plus, we encourage you to make a complaint.
You can submit a complaint using the complaints form, or by contacting the relevent person according to our complaints policy.
Please note: Shared Lives Plus cannot accept formal complaints about Shared Lives schemes, or Shared Lives carers.
If you wish to make a complaint about a specific Shared Lives scheme or Shared Lives carer, you should contact the relevent scheme directly, or speak to the regulatory body which covers Shared Lives schemes in the appropriate nation.
