Shared Lives Plus is working with more than 30 schemes across the UK to develop and grow Shared Lives as a positive option for care leavers, particularly those with special educational needs and disabilities (SEND). The long-term vision is to see Shared Lives become a core part of the offer for young people leaving care – alongside Staying Put, Staying Close and Supported Lodgings.
One of the schemes taking part in the programme is Telford and Wrekin Shared Lives, supporting young people leaving children’s care to make a smooth and supported transition into adulthood.

Kyle, a young person with Severe General Developmental Delay and Myotonic Dystrophy, transitioned from foster care into Shared Lives when he turned eighteen. Bernie, who had fostered Kyle since he was four, chose to become a Shared Lives carer so he could continue living with her – reflecting his preference and their long-standing relationship.
Planning for the transition began in the June, ahead of his 18th birthday in November. It was carefully planned and coordinated by his Personal Assistant (PA), social workers, and a multi-disciplinary team (MDT). Kyle was actively involved throughout to ensure the process was person-centred and aligned with his wishes.
The move into Shared Lives was designed to provide Kyle with both stability and opportunities to grow.
Bernie completed the required Shared Lives training and registration, while close collaboration between children’s and adult services ensured a seamless continuation of care as Kyle turned eighteen.
Kyle now receives tailored day opportunities and one-to-one support that focus on building life skills, social confidence, and independence – enabling him to develop at a pace that suits him while remaining in a familiar and supportive home environment.
Bernie says that supporting Kyle through Shared Lives has been both inspiring and rewarding:
“I wish I had moved over to Shared Lives a while ago. It is so inspiring and refreshing; every day differs, and I absolutely love my role.”
Her commitment has been key to providing Kyle with the consistency and encouragement he needed to thrive.
Shared Lives offered Kyle a stable, nurturing environment during a critical stage of his life.
The continuity of care from a familiar carer, alongside structured support, allowed him to gradually build confidence and develop essential skills at his own pace.

The combination of care, stability, and opportunities for skill development enabled Kyle to make meaningful progress while remaining fully supported.
Since moving into Shared Lives, Kyle has made significant progress in his independence and confidence.
He now takes part in household tasks such as cooking, washing up, and mowing the lawn, and has built strong social connections, including a close friendship with Chrissy, another person supported in Shared Lives.
Kyle has also enjoyed new experiences that have built his confidence and resilience – from quad biking and trips to Disneyland Paris to overcoming anxiety on a walking trip in Somerset.
Through these experiences and ongoing support, Kyle feels part of a family and valued community and continues to thrive.
Kyle’s journey demonstrates the importance of early planning and close collaboration between children’s and adult services to support smoother transitions for care leavers.
Allowing carers to continue supporting young people beyond the age of eighteen provides continuity and emotional stability, while Shared Lives offers a person-centred alternative for young people who might otherwise struggle in supported living or residential placements.
