Location: Based in the North East working closely with South Tees CCG, Middlesborough Council, Redcar and Cleveland Council and Avalon Shared Lives scheme to establish a new base in the region and support the expansion of Shared Lives services across the North East of England.
Shared Lives Plus is the UK network for Shared Lives and Home Share. Our members work on a small scale or in their own family homes, helping older and disabled people to live good lives in a place they feel they belong.
We are seeking an exceptional individual to support the development and ensure the success of our largest funded program to date. Scaling up Shared Lives in Healthcare is a £1.75million NHS England funded program to develop the successful Shared Lives model to support people with healthcare needs in the community.
This is a challenging and exciting role and to succeed you need to have experience of delivering services in the health and/or social care sector and have a good understanding of the current health and social care transformation agendas. You will be energetic, resourceful, resilient and able to work on your own initiative whilst working to a defined development plan. You will have experience of building and developing great teams and be a team player yourself. Most importantly, you will have the enthusiasm and commitment to develop the Shared Lives model for the benefit of people with health needs in the community you will be working in.
